Join us on April 16th for National Healthcare Decisions Day when we will be discussing what goes into long-term care planning, including appointing a health care proxy and understanding the legalities of a DNR. Most importantly, we’ll talk about how to get these difficult conversations started.

In 2018 Champions of Tomorrow a grass-roots 501 (c) hosted its first annual Breakfast for Champions to acknowledge individuals that service the community on a day to day basis and don’t always get the notoriety deserved. We raised 1,926.00 in our first year, that allowed us the opportunity to provide multiple rides to over 70 clients, consumers and patients to doctor appointments, employment and municipality meetings. Partnering with local human service organizations, providing services through our unique model of transportation called “Transitional Transit.” Transitional Transit services focused on supporting disabled and disadvantaged adults by providing them with the capacity to improve their lives through accessibility.

Individuals that utilized Transitional Transit services fell into one of the following categories; adolescents & adults needing transportation to and from job-training and skill-building programs; adolescents & adults receiving mobility training; adolescents & adults needing temporary transportation to and from employment; adolescents & adults needing transportation to and from local civic engagement opportunities; disadvantaged adults – 55 years of age and older – who are unable to utilize public transit.

With your support, we are looking to expand our services to the homeless population and young ladies aging out of the foster care system with no guidance of where to go or how to support themselves through our Champions of Independence initiative.
